Enroll in 401(k)
To enroll in 401(k), complete the 401(k) Enrollment Form and return the completed form in a SERV ticket to Human Resources. Please also complete the 401k Beneficiary Designation Form . Both forms can be found on the Human Resources HUB page .
Change Benefits Information
To change your contact information or beneficiary, complete the Benefits Information Change Form found on the Human Resources HUB page .
